PLEASE NOTE: You must provide a valid debit or credit card to hold your appointment time. If in the case that you need to reschedule or cancel your appointment, please contact me at 612-389-8787 no less than 24 hours before your scheduled appointment time to avoid late cancellation fees. Any cancellations made within the 24 hours of the scheduled appointment time will be charged 50% of the service cost (this charge does not goes toward any future appointments). Any no-shows will be charged 100% of the service cost. If in the case you are running late and have communicated with me, service will still be provided upon your arrival, however please note that your service may be cut short and end at the original scheduled time slot. If you choose to reschedule, you will be charged 50% of the service cost (this charge does not goes toward any future appointments). *** IMPORTANT PRE-CARE INFORMATION PRIOR TO ATTENDING TREATMENT *** Clients MUST BE OFF of Retin-A, Retin-A like products such as Tretinoin or Retinols (prescription or over the counter), or any active ingredients such as glycolic and salicylic acids, scrubs, or hydrocortisone at least 3 days (72 hours) PRIOR to your appointment. This is to avoid adverse reactions or increased sensitivity and irritation to your skin. Clients must be off of Accutane or Isotretinoin for more than 12 months prior to treatment. Appointments will need to be booked out at least 14 days following any derma-fillers or facial botox injections, unless provided a doctor's note stating otherwise. Do not wax any area of your face at least 10 days prior to your service. *** Please complete ALL FORMS + WAIVERS prior to your appointment ***
